Yes, in the country there is no mains water, and people have wells. You need a permission from the genio civile to drill. If you are buying a house with a well, it should be 'dichiarato' and legally exist.
Water in the south and Sicily is a big problem - it hasnt rained properly for 2 years. If its not a problem this summer it will be the next, or the next.

It needs to be tested at the outset, but then only if you're planning on drinking it should you need to do it regularly. However, turning on the 2 or 3Kw motor everytime you want a glass of water is an expensive way to go - personally I dont drink well water if it sits in a cisterna or a tank for a week before arriving at the tap.
