Default small extension to house?

We are in the process of buying a house near Francavilla Fontana, it's small. We wonder about adding a little extension (well within the 20% allowed) but I have read frightening stories of the enormous costs of employing a geometro - can anyone clarify this? To be clear this would be a very small box shape tacked onto the end of the house front with plenty of room beyond before the limits of the land.

Any extension to the footprint of a property needs all permissions and at least one professional to oversee the paperwork. You will also possibily need a geologist for the foundations. If you're adding under piano casa (the famous 20%) assuming you are in a zone where it is permitted, then you will still need to do all the paperwork and have director of works, health and safety and all the rest.
