Heading back to the UK
I am British and have been married to an Italian for 18 years. We both worked for the NHS for most of our working life ( me 30 years and my wife 40 years ) but we moved to Italy at the end of 2017. We now want to return to the UK permanently but have no idea which visa my wife will need to stay in the UK or if she needs a visa at all. Help would be very much appreciated...

Looks to me as though the OP's spouse would be eligible to apply for EU Citizen settled status in the UK :
Preparing to apply for pre-settled and settled status
To get settled status, you only need evidence for 6 months out of every 12 months for 5 years in a row. It doesn’t have to be the last 5 years. You can provide evidence for a different 5-year period - as long as you haven’t lived outside the UK for 5 years in a row since then.

EU spouses of UK citizens have until June 2021 to move the UK without a visa and apply for settled status.
