It took me about a month to get the problem resolved, and in my case it was a fairly obvious error. The results letter even stated in the text that they had given me the code that I had suggested on my application form, but the code was actually a different one.

Once they accepted the error, I had to send back the original assessment letter before they would issue a new one.
