Stat Dec wording for TRA

Stat Dec wording for TRA
One of my husband's past employers human resources department won't give a detailed reference for TRA due to the nature of his work. He worked as a prison officer. One of his past supervisors is happy to write a more detailed reference which we could use alongside the short reference his HR department has provided.
Would we include a Stat Dec with this to confirm its authenticity?
How is a Stat Dec worded and who can witness it?
