My kids are 5year and 2year.

There is a place on medical forms to sign by person who is under going the medical checks. I got two medical forms for kids as well.
Elder (5y) can write is name but younger (2y) can not.

What to do in the signature box for my kids ?
Is it ok to put finger prints ?

They do not need to sign, or at least thats what our panel doctor said. We have sent the off and "touch wood" they have not been rejected due to lck of signature

My son is nearly 15 and I still had to sign for him with x-rays, blood and medical tests....in fact i think you have to sign them in front of the appropriate medical staff so wait until you actually go for the tests.

My kids 6, 4 and baby were not required to sign. Think we signed for them.

My eldset is 11 and youngets is 5 and the Doctor got me to sign on behalf of the children.
