Hi Seniors,
Could you please provide me any template or any format on which I can ask my colleague to write reference letter for my job profile while applying to ACS for skill assessment.

I am currently In US. Can I notarize from any notary here in place of Statutory Declaration?

Your colleagues do not write your references. Your manager/HR person does. They will know how to format a reference.

Under some circumstances where references from manager/HR person are not available, ACS allows you to get statuary declaration from your colleagues, for the purpose of skills assessment...

And what about DIAC? Who's gonna write those? Wouldn't it be better just to get it now from HR?

Yes given in ACS faq.
27. I cannot provide a reference. Will you accept anything else?
If you are unable to obtain a Reference from your employer you should submit a Statutory Declaration. This is a written statement declared to be true in the presence of an authorised witness. This should be verifiable and be supported by other evidence of your employment (for example, by HR style references, recommendations from colleagues (with business cards attached), job contracts, pay slips and other relevant information that demonstrates employment).

ie he is not in a position to get it,that's why taking declaration route,
but for DIAC again there is provision , independent reference from his senior colleague can be provided.

What should we do if our manager/HR is reluctant to give any statutory declaration or we feel insecure after asking such question to our HR/Manager???

You have to decide about your own insecurity. The other situation where HR is not willing to provide a reference was described post #5 above.

Statutory declaration will do in most cases. Make sure an explanation is given that you fear to lose your job if you happen to ask for the reference or something like that. Of course, it would be left to the discretion of CO at ACS/DIAC, nevertheless, pretty much you can do from your side and hope for the best.
