I'd very much appreciate if anyone could offer help regarding this matter...

When applying for a visa, one usually waits with taking the medical until this is
requested, and the appropriate forms are submitted. If someone would want to include
both medicals and police checkups upon lodging the visa, how does one get a hold of
these forms...can they be downloaded from a site??

I've just made an appointment with an approved panel doctor and was informed that
they don't have these forms and that I will have to bring it with me.

Any help appreciated. /crank
