Medicine request: which docs doctor send and what do I send?
 
I got Medicine request letter and instructions are not very clear.

I am supposed to provide medicine, police and change of circumstances form.
Doctor sends medicine themselves, so I send other documents by myself? Do ASPC actually will receive two packages for this request?

Can/should I send copy of medical forms in my package? Or it seems that I am not allowed even to look at it.

And medicine request email says that "to speed up processing you're not required to send originals unless requested and can send scanned copies"

Doctor sends the medicals. You send the Police Clearance form and the 1022 to your CO. :)
