In a lot of cases you simply cant. There are rights of appeal with certain visa catergories where the decision maker reasoning is checked.
You can write to the head of the department section outlining your concerns.

Hi Thomas,
Depends what category of visa it was and what the mistake was. Please feel
free to email the details to me. I am reasonably well experienced in these
issues as we deal with about 40 appeals of various kinds each year. You do
have Departmental complaints mechanisms as Richard mentions, but the
Ombudsman is also an important avenue if there are no appeal rights.
Sometimes, where the error is blatant, a threat of a Federal Court appeal
will do the trick.
