Health Undertaking Form 815
We've received the forms asking for the medicals and in our letter from our agent he's asked us to fill in the Form 815 now to avoid a delay later if they find a problem at the medical.
Just sitting down tonight filling in all the medical forms and not sure what needs to be filled in on the Health Undertaking Form.
Nothing has been written in the weeks section ie how many weeks it should be before we contact them in Australia if they detect a problem -should we fill this in- do we need to put an address in Australia to be contacted at - also we haven't got flight details yet and they're asking us for these.
Is it simply just the case with this form to sign and return it and fill nothing in?

Hi Tracy,
We signed the forms and sent them back with all the other stuff, our agent suggested his po. box No. for a temp address.

Did you put any weeks in the box?
What about flight details?

Hi Tracy,
We left it blank, you should know whether they are needed when you get your medical done as the doctor will inform you of any problems. If there are no problems I cannot see the need for the form but it's better to send it to be on the safe side.

If you sign a health undertaking you might only be given 6 months to validate your visa from date of medicals. I seem to remember reading that somewhere.
Not sure if that's the same if you sign it even when you dpon't need it, or not, but why risk it, wait and see and assume medicals are ok, plenty of time to send it later.
