Default Form 47sk:Payment detail section

Hi everybody,

I was just filling up form 47sk and was just going through the payment
detail section(Part Q).

Since I would be paying by credit card, so I can tick one of the
checkboxes(mastercard, visa etc), put the credit card number, expirt
date, CardHolder's name, cardholder's signature as has been laid out in
the form.

However I didnt get the next 2 boxes wherein it asks to list the
telephone numbers and address. Now I get my credit card statements on
the e-mail instead of getting them delivered through physical mails
.Plus the fact that I have changed my addresses often.

So I am not sure which address and/or telephone numbers to put down
there.Is this information mandatory to enable DIMIA to charge the
requisite fees from my credit card. Or Can I just skip those 2 fields
OR just put down whatever be my present contact details.
Basically I dont want to end up in a situation where my application
gets stalled because of lack of payment!!Hence just wanted to make
doubly/triply sure.
Any pointers/info would be of gr8 help.
