Credit card number
 
Question No.108, requires details about the credit card.

Credit cards have 16 digit numbers, whereas in the application(Form No.47SK) there are 19 slots, to be filled up. I think the extra 3 digits refer the identification number of the card, should they be entered as well or is it alright if one enters only the credit card number without the card identification number, in which case the last three slots would be blank?

Not all credit cards are created equal. Visa and Mastercard cards have 16 digits, other types of credit/charge cards may have more or fewer digits.

I think I put dashes in between each block of 4 digits.
