Confusion re. Forms for 2nd stage of Spouse Visa

Confusion re. Forms for 2nd stage of Spouse Visa
Okay so we got the forms from DIMIA in Canberra for the 2nd stage of the Spouse Visa. We are bit confused as to exactly what they want when it comes to our statements.
They have sent two forms called "Statement by Applicant" and "Statement by Sponsor". These are fairly simple forms and at the end there is a box to sign and a box to date. So far so good.
But when we read the instructions contained in the covering letter it doesn't mention these forms at all - instead it refers to Statutory Declarations from the applicant and the sponsor. The checklist also has no mention of these forms and again asks for Stat Decs from both of us.
So what do we do - fill in the form, or get a formal Stat Dec done or do both ?? The form does not state anywhere that it is a Stat Dec (unlike form 888 which our friends have to complete) and it has no space or boxes for a witness to sign and date (again unlike Form 888 which has boxes for the witness to fill in).

We just filled in the forms provided. The agent told us the most important bit was the stat decs from friends, as they check 2 years on if they will still vouch for you. Don't worry about getting "proper" stat decs for you two.. just use the forms they gave you.
