457 and insurance

Hi
I have a query about hte insurance for a 457 visa. I understand that we have to get it in advance. I'm going to go through Medibank just because we have friends who use them and they seem reasonable. However
1. we have to show a letter that we have the insurance but can the insurance be arranged to run from a later date or must it start from the time of visa application.
2. do we have to get overseas visitor cover or can we just get the bog-standard cover that any other person would get.
I will phone to check but I just want to get my head around what I'm doing beforehand.
