Yet Another Medical Question....
 

Sorry but here are another couple of questions....!

1. The Medical and X-Ray form (1007) - Is a downloaded form from the internet acceptable, or must you obtain a genuine form?

2. What sort of answers have people put down for "ever received hospital treatment?" I have had ENT, a tooth extraction, cast on a fractured arm and dysentry all treated in hospital. Should all these be declared?

I'm a little worried about the dysentry - albeit 17 years ago!

The downloaded copies are fine,that's what I used. I put down everything just in case,including wisdom teeth extraction under a GA 15 years ago. The doc in Glasgow asked if any probs now and when I said no wrote as much on the form. Hope this helps

We also used the downloaded forms, I wouldn't worry too much about things that happened a long time ago if you have made a full recovery, I think it's only serious problems with long term effects that they're worried about.....
