Shall I include this in my NZQA Application
 

Hi every one,

Quick question pls..:)

Is it mandatory to submit an Identity document (e.g. Passport Copy or Birth Certificate) along with my other documents for the NZQA?
Pls advice??

Hi,

An identity document is something that is listed to include with your application. I sent my marriage certificate which was more appropriate for me as it showed my maiden name before marriage. This was one of the questions to be answered when filling in the application.

I would suggest that you do include an identity document that you feel is suitable to your case as it would be disappointing to find out that your assessment is being held up because they have not received all documents required.

I sent my birth certificate as that is still the surname I use :)

Thanks for the reply :) Any how; i could understand that we need to send an Identity Doc. in case if any name change only?
