Hi All

I have the great pleasure of filling in my NZQA forms at the moment but I am at a total loss when it comes to the transcripts that I am meant to provide. I have got all my secondary school exam certs, my A Level certs, my degree cert and my Post Grad cert. The only thing is, I never received a "completion of school certificate" which they ask for and I have no idea what an index book, a diploma supplement is in relation to my qualifications. Could anyone help me out on this one?
