Daft medicals form question

Default Daft medicals form question

In order to pre-prepare for our ITA, I'm getting together our policer check application forms, and medical forms.

I know I can download the form from the immigration website, but when printing it can it be:

single sided,
black and white,
should it be stapled / paper clipped?

sorry .. jut want to get it "right" so immigration have nowt to delay us with.

Hi Lou

We downloaded ours and printed it in colour, one sided and I seem to remember putting a paper clip on each of ours.

You must remember to also provide some passport photos, lots of them by the time you put your ITA in, so worth getting them done by a photographer
