Default wrong address on medical

hi all

i just had a major alcaration with the housemate of the unworkable kind, and i'm going to be moving again...

i just got my medical request, booked for 2 june. The paper has the wrong address on it, my previous address, though the envelop had the correct one.

What should I do? I'll be moving in or around the same time as the medical, but that will then be almost 2 addresses ago! can i just cross it out and fill the correct one in?

Medical forms are printed at the early stage of the process and subsequent address changes won't be incorporated into them. Nothing to worry - you already know that address change worked as envelope was addressed properly. You may ask DMP to correct the address on forms, although it is irrelevant at this stage.
