Where can one apply for their health card once they land in Canada?

Health care is administered by Provinces - so, it is impossible to answer
your question without knowing where you will establish residence. You need
to establish residence first to qualify anyway and in most Provinces you'll
have a waiting period (during which you must live in particular Province to
be eligible for health care).

You will be in Ontario, right? Office locations are listed here:

You probably can't apply until you have your driver's license or a copy of a lease agreement with your address on it though.
The form you need to fill out with information about the required supporting documents (proof of permanent address in Ontario) is here:

You can fill this out online and print it.

I used a bank statement (from my account in the US) with my Ontario address as proof of residency. To apply for a health card go to your local branch of the Ontario Health Ministry.
