Visa issue till landing time-frame?

Hi,

I was told at the embassy that I would receive a landing permit/visa about three
months before the expiration of my medical examination's results. Could someone
please clarify for me:
a) Does this mean that I'll only definitely know that my application has been
accepted 3 months before I have to leave for Canada? This seems very little time
to prepare to leave a country (leaving a job, an apartment etc'). Will I get some
sort of early notice before the landing permit?
b) Is this the standard time-frame or does it depend on the country of application?
