I have done a search and can't find an answer to this question. Can anybody confirm that form 3019b is the correct one to use? There is a 3019a which is similar.

I don't know what the form number is but it is the Data Protection Act, Subject Access form.

Thanks Tiaribbon, unfortunately both of the forms (3019a and 3019b) are under the Data Protection Act.

OzRob, I am 99% sure that it is form b.
I can't find the link at the moment to look, but I'm sure that it is!

But have just checked, and it is definitely b.
I got mine back very quickly, I got a receipt from them 2 weeks after I had sent it, telling me to expect it in 4 weeks.
Two days later it was in my mailbox!

I was surprised that it got to Oz so quickly!
