Strange medical forms

Default Strange medical forms

Dear friends,

We applied for immigration in Moscow, but for three years now we live in France.

Recently I received some medical forms and instructions.

I write 'some' because they differ from what I downloaded from internet.

I have got from the Embassy only the Sections A which are to be completed by
the physician. There is no the Sections B-D. Is it possible that they were not
sent by error?

These Sections A are already with our photographs which are 3-year old. We are quite
different now.

In addition, the name of my son is spelled there as Rouslan instead of Ruslan
according to his passport.

Before going to the physician, I would like to know if it is OK to go there without
the sections B-D and with that wrong spelling of the name?

Go to the medical examination with the forms given and explain to the DMP that there
is s spelling error in the one form. Jim Humphries, former visa officer
