sending pictures & passport
When sending pictures & passport, should we still put applicant's name, DOB, file # and date of picture taken on the back of the photo??? It's not indicated in the letter.
Also, do they require height and eye color for everybody? It is in the letter but the box next to it it's not checked out. Should we volunteer this info?

For each person in your family, fill out the name, date of birth, height and eye color. Also do not forget to write on the back of each photo the name of the person it belongs to.
