Default Risks of medical test

Hi,

Can somebody explain to me what are the main risks of the medical test for the
independent immigrants? In other words, who has a higher chance of getting rejected?
For example, if someone in my family has a heart defect (not life threatening), would
that present a reason for concern? Is there anything special that must be shared with
an immigration doctor, or any documents that he/she typically requires? Does anybody
know people who failed the medical test and why?
