Default Questions about "Statutory declaration of common-law union" form

This is a question about Form IMM 5409 "Statutory declaration of common-law union"

(Q1) The form suggests that before you start, you read the "instruction guide". I have looked for this and have not found it. Anyone know a source of this online?

(Q2) The first line of the form says "Country" and "Province/State..." What do they want here? The country where the form was filled out?

(Q3) They seem to only want you to fill in section (4) with proof of your relationship if you have not checked something else. Do they really only care if you have a lease together and have a joint bank account?
