Proof of Medical Examination Question

Default Proof of Medical Examination Question

My husband and I just received the Proof of Medical Form 1017 that we must submit with our package for a Spousal Application for PR. The Doctor did not attach a photograph to the form although he was provided with 4 at the time of the examination - should have have done this or is it is normally submitted without?

IF you went to a DMP then it is likely that all was done correctly. Unless
you were present at all times you might well not see what they do before
they send off the results.

Thank you for your reply Mr. Humphries. We did indeed go to a DMP as designated for his state. My concern is that Dr. initially did not sign the form, we had to go back and have him sign and date it. That is why I wanted to double check if he was supposed to attach one of the 4 photographs my husband provided him with.

" A Medical Report, Section A (form IMM 1017 EFC) will be completed at
the doctor's office for yourself and each of your family members
undergoing the medical exam. A photo of the person to be examined by
the doctor will be AFFIXED on both copies of that person's form. This
form is used to match your medical file to your immigrant file ".

Hm Okay
Well I guess the Doctor screwed up then. We had to sent the form back to them once to have them sign it and they didn't attack the picture. The package has gone in I guess we'll just have to wait and see what happens. I am pretty annoyed at the Dr. as they are the only ones approved for the State my husband lives in.
