pink copy of a medical form

Hi all,
Am I suppose to send in the pink copy of medical form with my photograph attached to CIC?

You are supposed to take the form to the doctor who will do what is needed.

Sorry for the confusion. What I meant was the doctor return a copy of medical form (section A) to me after the medical exam. should I send that copy along with other documents to CIC?

If you are a sponsored spoouse then you should include it with your application package.
