Default photographs

3 months ago i was requested additional docs and photographs, now i received a passport request for visa stamping and they still need more photographs - but they didn't mention how many........
is that a normal practice to ask for photos when passport is stamped, or that's because the photographs I sent last time are unacceptable (i sent them 3.5/4.5 pics and exactly the same as specified in their instructions- plain white background..etc).
when i send the pics again, should i write my name and other info on the back side, or just leave it blank? thank you

Send them 4 each, write your file number on the back.
