Default Passport name issue

Hi to all the forum members,

My medicals reached London 18th Dec, 07 & I am awaiting PR.

I have a query regarding my name on my passport.

My name is as follows:
Surname : D'Gama
Given Name : Vincent

The name on my Passport reads as:
Surname : ------
Given Name : D'Gama Vincent

I applied to CHC as was mentioned on the passport.

When I get my PR alongwith some forms where we again have to fill in
the details, should I;
1) Write down my name as was given to them on the passport
2) Write down my name as it is i.e. Surname: D'Gama Name: Vincent
3) If I get it corrected on the passport now, separating my Surname
& Given Name, will it create a problem at the time of visa stamping.
4) Leave it as it is.
5) If I leave it as it is will it cause a problem at the POE or will it cause
any future hassles.

Please members give me your expert advise & put me at ease.

Thanks & Regards.

Correct it in the passport to avoid future problems - as your PR Card and everything else will be based on what info you provide now. Why didn't change it yet anyway?
