Ordering CAIPS notes?

Default Ordering CAIPS notes?

I'm sponsoring my wife (outside) and would like to find out what is
happening with our application. Does my wife (principal applicant) have to
sign the CAIPS consent form in order for them to send the file to me, or is
my signature good enough for both of us?

Thanks! I think I understand now...

As a Canadian citizen, I can get the information on her behalf. So I have
her sign the consent form (faxed copy okay?) then send it and form "TBC
350-58 - Personal Information Request Form" along with a $5 check payable to
the "Receiver General" to:

Public Rights Administration
Citizenship and Immigration Canada
Narono Building
360 Laurier Ave, 10th Floor
Ottawa, Ontario
K1A-1L1

Should I be the one to sign "TBC 350-58" or should she sign that one too? (I
want this mailed to me, since I'm here in Ottawa)

Where can I find form TBC 350-58 for ordering caips notes. I was able
to get the format for consent form.
