Hello all!

I would like to ask for your assistance regarding the medicals.

I applied in Buffalo for immigration (skilled worker). I received the
medical form yesterday. However, the instructions are not very clear
to me.

Could you help me with the following:

1. How much is the fee for the medical exam?
2. What form of payment can I use?
3. What number do I call for making the appointments for the exam?
4. Do I pay for mailing the results of the exam to Ontario, or is this
included in the fee?

You should have received list of doctors

You pay doctors directly (cash)

Check with doctors as to fees

I just enquired as I need to do mine

$100 CDN per medical for each person
$33 CDN for xray per person
$8 - $10 CDN for blood tests

Doctors mail everything as far as I know

The form said this

You also need to take A1 forms

and 3 photos per person (if A1 form has photo) or 4 photos if not

This information is based on CHC London but assume the same for Buffalo

You should have received a list of approved doctors including their
addresses and telephone numbers. Only the doctor can answer your questions
1, 2 and 4.

Go to http://www.cic.gc.ca-
/english/contacts/dmp/usa.html
and find a doctor
near your area. These doctors are called DMPs (Designated Medical
Practitioner).

The fee depends on the DMP as well as the tests needed (you may need some
extra tests). It is usually around US$250 - US$350.

Some DMPs send the results via regular mail. Make sure that you force them
to send your results to Ottawa by courier EVEN if you have to pay the
shipping fee extra.

My timeline:

Sept. 24 - applied in Buffalo (skilled worker; biological tech)
Dec. 3 - received AOR
Sept. 28 - received medical forms, interview waived
