London: Baby arrived during PR application What Now?
 
Done a search for baby related posts but didn't find what I'm looking for. Sorry if this question is a repeat post.

UK resident applying for Skilled worker PR through London CIC.
Medical request received 2 months ago but put on hold as wife was pregnant so advised CIC London by mail that their will be a delay in completing medicals.

Baby is now 4 weeks old and we are ready to book our medicals, but the doctors receptionist mentioned we need a form for the newborn baby.
What is the form called and where do we get it from?

You need to send the following to visa post processing your application:

1. Updated application forms - IMM 0008 and IMM 5406

2. Baby's birth certificate

3. Baby's passport (copy)

4. Required number of baby's photos in proper size

5. Application fee of CAD 150

with cover letter notifying visa post about addition of new baby and requesting medical forms for baby. Don't forget to provide principal applicant's name, date of birth and file number in cover letter.

Congratulations on new born! Provide items Andrew mwntions and CHC will send out medical forms for baby. If you haven't already done so, make an appointment to get babys birth certificate and passport. There can be significant delays on getting these.

Hi,

You need the full version of Birth certificate which shows both mother and father names.Our baby was born during the third week of April and we have sent the all required documents to Buffalo, USA on May 03 and we are still waiting for Medical forms.

Andrew, Do you have any idea how long it takes to get medical forms for baby after sending forms. The money order was cashed on May 09.

Once baby documents are in order we will be back on track with our PR application.

Mine took about 4-6 weeks. I sent the info in early October last year when I was in the UK and they posted out the form to me in Dubai early November.
