IMM5669E - do I put 'present' or date it

Default IMM5669E - do I put 'present' or date it

I have to send off a bunch or forms again (hopefully for the last time), one of them is the IMM5669e form but after it's listed on the email it says 'this form must be fully complete. Each month must be accounted for' which suggests to me they're not happy about something. I've looked over it & the only thing I can think of is that I put 'present' in boxes for things like address & employment, I thought that was acceptable but am I wrong? Should I just put in the date I sign the form with?
