IMM1344 form won't open online

i've downloaded most updated adobe reader
still no joy

any pointers?

also concerning medicals can i only go to a Panel Physician which is on the cic site?

Open with another browser?

I use Chrome all the time and it didn't like a lot of the forms. Changed it to the big blue e (Internet explorer) and it worked fine :)

I had the same problem - I was going to attach the form but just realised that they have a newer one now.

Yes, you must go to a designated medical practitioner (Panel Physician). You can find one near where you live by using this tool: http://www.cic.gc.ca/dmp-md/medical.aspx
