Default Got Passport Request. Wrong address and deadline to land

Hi all,
Finally after waiting for 3+years, I finally got the Passport Request from Buffalo IRPC. I am an Oct 2001 applicant.
Thanks for everyone especially Mr. Miller and Mr. Humpshire for all the help and advices.
I will be sending the passports in a few days and I have a few questions:

1) The grey envelope which I am supposed to use to send the passports already has the return address. The address is wrong and I had moved from that place around 3 years ago. Interestingly the they mailed me letter and envelope at the right address but put wrong sticker in the grey envelope.
Please advise what should I do?
Should I remove that sticker and put a new one with my correct mailing address?

2) The medical results are valid till April 2005 as we did our medical tests in April 2004. Is that right?
What will be my deadline to land in Canada?
Can I extend it somehow till Aug or September 2005?

Experts, please reply!

Thanks.

Experts I have another question:

3) The latter is dated Sept 3, 2004 but was posted on Nov 30,2004. The latter gives me a deadline of 30 days. If I start counting about from the date on the letter then the deadline has expired 3 months ago.
Should I send the passports with the explanation that I received the Passport Request 3 months after 30 day the deadline expired?

Experts please reply to my questions?

Thanks.

Check out ur and ur dependent passports who will travel to canada. if any one is going to expire before April 2005, then that will be ur final date for landing other wise April 2005 untill medical expiry. there is no way to extend
visa expiry after issuance.
