Got Passport Request but cannot finish it yet

Got Passport Request but cannot finish it yet
Got passport request letter last saturday. But I cannot send our passports yet because my child was born two weeks ago. I will send some additional documents to add my baby to our application. Here is the list I can come up with at the moment:
1) Certified copy of Birth Certificate;
2) Copy of Passport;
3) the second page of IMM0008;
4) 110 USD application fee;
5) 4 photos;
6) cover letter to inform CIC the birth of child.
Anything else?
My timeline:
applied in Aug, 2003
AOR in Sep, 2003
IA in May, 2004
Medical in June, 2004
Additional Docs in Aug, 2004
Passpot Request received on Oct 9, 2004

In the cover letter ask to add child to your file and to send you ASAP medical forms for the child.
