Is it a stuffed Dog ? ( then yes ) Is it a live dog ? ( then No ) it is a seperate form and costs $35.00 as I recall

I Used a pet company, they did all the paper work.

No but you must include your dog as a dependant family member on your application, and they must also have a medical and police checks.

Without the above, your dog may be banned from entering Canada for life.
