FBI, fingerprints from Poland

Hallo, I used to live in USA. Now, I'm in Poland and I would like to immigrate to
Canada so I need FBI clearance. How can I get fingerprints form and where should I
get fingerprints? Can I use any form (for example from local polish police station)
or do I have to take any special form from FBI? Any advice will be highly
appreciated. Richie

FBI will accept any police form however if it is in Polish you should send a
certified translation . Check with the US embassy to see if they can supply forms.

You may use any fingerprint form which allows for a full set of fingerprints.
The local police station may be able to do the fingerprinting on the form that
they provide.

The remaining process may be done by correspondence. The address and instructions are

US embassy in warsaw does fingerprints.
