Fax number for Buffalo
Hi,
We recently received our med requests, but my daughter's middle name is missing from Section A of the IMM-1017 form.
As per the instructions, I am trying to notify Buffalo of the error by fax (they say to use only mail or fax). Unfortunately, the fax number is constantly engaged (been trying for 2 days now).
Does anyone know of an alternative fax number for Buffalo?
Thanks

If the fax is engaged, then send a case specific e-mail to their direct e-mail at [email protected]

Cheers, will send an e-mail, would have been easier by fax as I could have sent a photocopy of the form with the error on.
To be honest, I would have just left it as is, but my wife says no, let's not give them a reason to delay things, particularly when they issue instructions to check the forms as knowing our luck, they would pick up on it.

There are two fax numbers for buffalo, one which is always engaged/busy, the other which works. I have been informed that both are acceptable to use:
(716) 858-9591 (Busy one)
(716) 852-2477 (Used this one for an address change and it got to them OK)
