Child birth notification form for Buffalo

Default Child birth notification form for Buffalo

I had my child born recently and want to notify Buffalo by fax
regarding the child birth. Could somebody point me to the form I need
to fax. Would appreciate help asap.

Can I just call up the Buffalo center and notify them over phone
regarding the child birth. If a fax is required, please let me know if
their is a standard format for the fax form for Buffalo. Appreciate
your help as always.

The question is why do you want to notify Buffalo in the first place? Are
you a Canadian citizen who wants to register birth abroad or are you a
Canadian Permanent Resident (in such case baby needs to be sponsored once
you are back in Canada) or do you have Canadian PR application pending in
Buffalo?

You are not getting any answers so far as you didn't explain the
circumstances at all.

Sorry for no giving all the relevant information before. I have
applied for Canadain Immigration in the Independent Skilled Category
in Dec 2001 at Buffalo while presently residing in US. So far have
recieved the inerview waiver and the medical forms for me and my wife
to be done after 6 months i.e. after 12th Aug 2003. My child was born
recently and I would be sending the documents for his addition in my
application in 2 months time ...have to wait to get the passport, SSN,
birth certificate etc. But meanwhile want to just notify Buffalo about
the child birth informing the center that documents/fee would follow
in 2 month's time.

Hope you can now help with my querry which is as again pasted below:

In such case send a fax to Buffalo notifying them about the fact and asking
for instructions. Or do it all yourself - complete updated IMM 0008 form for
yourself (including this time your child) and updated IMM 5406 for yourself
and your spouse. Submit forms with the notification, child's birth
certificate and CAD 150.00 application fee to Buffalo and don't forget to
ask for medical forms for your child. Passport may be submitted later.

Again - there is no standard form of notification, just write a letter in
your own words.

Thanks for your reply Mr Miller. Its been very helpful. Please help me
with these additional querries. I am an IT professional with 7 yrs of
experience with Bachelor's in Engineering presently in US on work
Visa:

1) Since you suggested I send the IM0008 form with the updated info
about my family, do I need to send all the following filled forms or
only the first one:

a) Application for Permanent Residence in Canada [IMM 0008]
b) Background/Declaration [IMM 0008 SCHEDULE 1]
c) Schedule 3: Economic Classes [IMM 0008 SCHEDULE 3] -For Federal
Skilled Workers

2) In the IM0008 form page 1 top left corner, would the category
that'd apply to me be "Econmic Class". Please confirm.

3) Wouldn't I have to wait for my child's passport before I can send
the forms, as the IM0008 page 2 needs Passport details of the child.

Please help.

1) The only updated forms needed when adding a child are - Application for
Permanent Residence (IMM 0008) by principal applicant and Additional Family
Information (IMM 5406) for principal applicant and dependent spouse.

2) Yes, economic class

3) It would be better to wait for the passport.
