I have sent my documents on 23rd of march and only waiting for my police clearance from Tehran. Can I do my medicals while I am waiting for this letter?

No, you have to wait for the medical request, unless you are a spousal applicant.

We have not yet put anything in, but are in the gathering process. My only concern is that I had surgery on my spine (two rods for a curvature) which is not noticeable and has not affected career (nurse) or life in general. I wonder if I can speak to a Home office med person to clarify if immigration is possible at all rather than waste lots of time and money?

Any thoughts?
