Default Bankers draft and paying fees - name and address

Hi guys,

I'm sending off my full documents to CIC in NS in the next day or so for a PNP application. I've filled out the payment form but I'm unsure about the name & address of payer (if different from principal applicant), bit. I'm paying, but since it's a bankers draft, it has my bank's name and address on it. Do I leave these fields blank or do I put my bank's name and address in these fields?
