B4 E Forms: 1 per couple or person?

B4 E Forms: 1 per couple or person?
Happy Holidays!
I had a question regarding the B4 E form (personal Effects Accounting Document).
We'd like to fill it out ourselves in advance so that it will be both easier for us and Customs, but I am wondering if we need to have 2 forms filled out.
The reason I ask is because at the bottom of the form there are 4 boxes:
1: Former resident
2: Beneficiary
3: Seasonal Res
4: Settler
We are both box 1 and 4. Can we cross off both boxes on 1 form, or cross on box each, one on each form?

Hi
2 forms, ensure that the Former resident amounts don't go over 10K for anyone item, see: http://www.cbsa-asfc.gc.ca/E/pub/cm/.../d2-3-2-e.html

I am the former resident, we're only taking clothes and dvd's - no electronics except computer so I think we'll be ok in regards to the " no one item over $10,000." Thanks for that reminder as well.
