Default Application to extend stay in Canada (Andrew or Jim)

Hi

With the application to extend your stay in Canada form:

http://www.cic.gc.ca/english/applications/visitor.html

For the family members section - do you have to fill this in for parents, siblings etc if they are not in Canada with you? My work colleague is currently on a work permit that expires on 17th Jan and he wants to extend his stay as a visitor for 2-4 weeks before moving to Australia. He's filling in the form today. Also, he is living with his girlfriend in Canada (she's british and on her own separate work permit), should he put her on the form as common-law partner or not, they have lived together for less than a year and she isn't involved in his application.

Also do you know if you can fax this form in?

1. No, he is only applying for an extension for himself.
2. No, they aren't c/law so it doesn't matter.
3. Mail.

Sorry, one more question. For ID, is a photocopy of his passport ok or does he have to send off the original?
