All Experts and Specially Andrew, Thanks for the information being dished out. Appreciate
it much. Here my situation : I have all my documents ready, including the FBI
clearance..(Took me a little over a week, about 10 days to get that) If I understand
correctly this is what I need to do..
a. Take 3 copies of the printed form and sign them. Copies of the signature wouldnt do..
b. Do I also need 3 copies of the 5406 form (the one where you include family details)
c. I have all my certificates etc..and documentation enclosed.
d. Do I also include copies of my passport (old and new)
e. Anything else I am missing out here...
