Default Additional family information

Hi there,
Can anyone tell us if we need to fill in an IMM5406 form each or just the applicant, as a skilled worker applicant. It is a little unclear as the notes say it must be filled in by the applicant, spouse and anyone over 18. Not quite sure where section C comes into it with putting brothers and sister info on there, is this just the applicants?

No its both you and your wife, I see your son is 14 so he is exempt. on your one you will put your name as the applicatant and your wife as your spouse, in section C you will add your brothers and sisters.
Your wife will complete one as her as the applicant, you as her spouse, and in section C she will provide details of her brothers and sisters.
