Do we need to write in brothers or sisters who have passed away?

Yes - just write Deceased in their marital status and N/A in their address.

Thanks, new question, form 5540 question #9....I am a naturalized Canadian, so I answered yes, and then it asked what date I became a PR, not sure I Know that. The date I became a Canadian citizen I have.

Have you ever been a PR or not? If not, then you just put Not Applicable.

I came to Canada when I was 6 (1966), so we were landed immigrants first, and then got our Canadian citizenship in 1973.

OK, so when you moved to Canada you became a Permanent Resident? So just put 1966.

Okay, new question...which visa office issued our immigrant visa or confirmation of PR? I have no idea.

My dad says the Canadian Consulate in the Hague, in the Netherlands.
