ARTC Question 9

Hi everybody need some advice. My Ex- employer won't give me a verifiable statement on headed note paper about my employment history and work under taken during that time, despite being asked twice and sending them all the stuff on disc just to be printed out and signed. All I've received off them is a short 5 line reference. which wasn't what i was looking for, can I just send a affidavit with the attached letter. Please advise :(
